Determining best practice for safe discharge of the older emergency patient.

Study 4

Validation of risk prediction tools

This study aims to evaluate the effectiveness of two risk prediction screening tools for unsafe discharge:

  • the Identification of Seniors at Risk (ISAR), and
  • items from the Alfred’s Initial Nursing Assessment Tool (INAT).

Study 4 will be a prospective validation study of the effectiveness and clinimetric properties of the ISAR and INAT items for predicting and preventing unplanned re-presentation / admission within 30 days of initial presentation to the Alfred ED/AMU. Patients recruited in the prospective cohort study will be involved in Study 4.

Unsafe discharge is defined as unplanned emergency re-presentation or unplanned admission within 30 days of the initial emergency index presentation to Alfred Health.
