Determining best practice for safe discharge of the older emergency patient.

The growth in the number of older patients presenting to emergency departments is a major concern and is impacting on acute hospital services and patient flow.

Initiatives introduced to manage these patients have not been formally evaluated and the impact remains unknown.

This project aims to improve care for older patients presenting to Emergency Departments or Acute Medical Units by:

  1. Reviewing best practice evidence of emergency models of care;
  2. Prospectively mapping the care journey and health outcomes of 1000 patients, aged ≥65 years, presenting for short stay emergency episodes;
  3. Evaluating risk screening tools for unsafe discharge in an Australian cohort of 1000 patients; and
  4. Developing an older patient short-stay framework of emergency / acute care.

This project is of operational and societal importance and results will influence local and international practice.
